What do I do if I forget my Duo device at home or if it is lost/stolen/destroyed?

Forgotten Device
If you still have the device needed to authenticate, but are unable to access at this time (IE. you left cell phone at home), contact ITS to obtain a one-time bypass code. Passcode will be valid for 60 minutes and is for one time use. Checking the remember me for 30 days box will let you in sign in without using the passcode every time. The remember me for 30 days both browser and device specific.
If your device has been lost/stolen/destroyed, you may need to purchase a security token from Information Technology Services.
Lost/Stolen/Destroyed Device
If your authentication device has been lost/stolen/destroyed, contact ITS for a temporary passcode so you are able to access your BGSU accounts. The one-time passcode will be valid for five days and will not be reissued multiple times. To provide additional time to locate/repair/replace your Duo authentication device, check the Remember me for 30 days box on the Duo authentication screen.
