What is it?
The process for submitting a request to remove an existing VoIP or analog telephone service, typically when it is no longer needed or an employee has left the University.
Please Note: Phone equipment returned to ITS without a ticket will not be removed from billing.
How to request it?

Your department's budget administrator's approval is required before filling out this form.
How to use it?
