What is it?

The process for requesting access changes for an EvaluationKIT department account. Changes can include adding or removing staff as well as changing hierarchy level assignments.

Use this form to request access or hierarchy level changes to the University-wide Evaluation program or an EvaluationKIT account managed by your department. Do not use this form for instructor access as this access is managed internally in EvaluationKIT.

Where to get it?

Please complete the Request Change form on this page. 

By submitting this form you are certifying that you are the authorized representative to manage course evaluations for the academic unit requesting the account, or have received authorization from the appropriate party to make this request.

How to use it?

Use the form to request additions and removals of administrators for an EvaluationKIT account.
