What is it?

This service is used to report a software issue for a computer lab or classroom.

Service requests may be made by clicking on "Report Issue".

How to use it?

While completing the service request form, please be as specific as possible.

•Include a detailed description of the issue, steps needed to replicate the issue, and attach any supporting documentation, such as screenshots.

•If the location is a computer lab, select the location from the list of lab locations; if the location is a classroom, enter the building name and room number.

•If an entire lab is not impacted, include the BGSU asset tags of the impacted computers.

•ITS will seek to resolve the issue within 10 business days.

•A new service request form should be submitted for each individual issue; please do not lump multiple issues into one service request form.
