Accessing Microsoft Forms

How do I access Microsoft forms?

Microsoft Forms is an Office 365 application for creating digital forms, surveys, polls and quizzes. Forms can be sent to users to complete and the form creator can then review the results online. View this Microsoft video  for an overview of the application.

To Access Microsoft Forms:

1. Navigate to https://forms.office.com/.
2. Select Already have a Microsoft account? Sign in.
3. Log in using your BGSU email address and password.
   • Select Work or school account if prompted. 

If you are already in an Office 365 application, you can access Microsoft Forms by following these steps:

1. Select the App Launcher menu from the top left corner of your screen.
2. Select Forms from the list of applications.
