University Device Authorization Acceptance Details

What is the University Device Authorization Form and what does it say?

Answer

As part of the University Device Request process, users must review and accept a Device Authorization Form prior to receiving their university device(s). The Device Authorization Form states the following:

In order to move forward with this device request and for ITS to reach out for scheduling, you will need to respond “yes” if you accept the below statement.

The MAKE/MODEL device, with serial number XXXXX and BGSU Asset Number XXXXX, is being supplied to me by Bowling Green State University for my work as an employee at the University, per ticket XXXXX.

I understand and acknowledge that the device was purchased with state funds and has been inventoried by BGSU Information Technology Services (ITS). I acknowledge that I am not to transfer possession of this device to any other individual without first contacting ITS.

I acknowledge that my use of the device is subject to all BGSU Information Technology Policies. I acknowledge that I am responsible for maintaining this device in a state of good working order while it is in my possession.

I understand and acknowledge that I or my department will be held responsible for any costs related to repair or replacement of the device and/or provided peripherals as a result of theft, loss, accidental damage, or damage not covered under the device’s warranty.

Should Bowling Green State University provide notice, at any time, that the device must be returned to the possession of ITS, or should I leave employment of the University under any circumstances, I acknowledge that I am responsible for the return of the device in a state of good working order.

I acknowledge that my acceptance of this device serves as my agreement of the terms above. ITS will contact you for scheduling upon an acceptance of "yes" of this form.
